"Application" is an application used by PKU Muhammadiyah Surakarta Hospital to display the status of Corona Virus Disease (COVID-19).
"User" means any person who uses the application.
"Personal Data" or "Personal Information" means any personal data provided by the Application User, including but not limited to name, identification number, health data, user location, user contact, and other documents and data as requested on the form. Account registration or other forms when using the application.
The application displays Personal Data to display the COVID-19 status, managing and expediting the process of using the application and other purposes as long as it is permitted under the provisions of the applicable laws and regulations. Personal Data collected is as follows:
Personal Data includes patient medical record numbers at PKU Muhammadiyah Surakarta Hospital, Population Identification Number, Place and Date of Birth, and Patient Status. Data obtained from other sources, including but not limited to government or private institutions based on laws and regulations or Personal Data sharing agreements in connection to the application's use.
User's data recorded will be stored in the application that has gone through a verification process and is encrypted as long as the service operates.
If the application is not used or its utilization is discontinued, the application continues to store and use the User's Personal Data following the provisions of the legislation.
The application uses the infrastructure & server owned by PKU Muhammadiyah Surakarta Hospital to store Personal Data and legal software to obtain and collect Personal Data.
USE OF PERSONAL DATA
The application may use the Personal Data collected for purposes including but not limited to:
The application will process and analyze Personal Data to display patient status about COVID-19 in Indonesia.
The stored Personal Data will be processed for needs, including:
Save and display the history of the User's covid 19 status.
SENDING AND DISTRIBUTION OF PERSONAL DATA
User's Personal Data will not be sent and disseminated to other parties, except for Legitimate requests from law enforcement officers based on laws and regulations.
The parties based on the agreement will use the Application data to support the efforts of the Government of the Republic of Indonesia in tackling Covid-19 and other infectious diseases.
DELETION OF PERSONAL DATA
Since the application is not operating, personal data will be deleted from the Application server for at least 5 (five) years since the application is not operating.
For the security of the User and the public, the User is expected to grant access permission to the User's device. Users can cancel permission to access the Application device through the settings menu contained in the User's device.
ACKNOWLEDGMENT AND APPROVAL
The User declares the Personal Data submitted to the application at account registration or m. renewal is accurate and up-to-date data.
If you are under 18 years of age and unmarried, the User represents and warrants that the User has obtained permission from a parent or legal guardian. The User's parent or legal guardian agrees to comply with these Terms and Conditions of Use.
If the User finds a security gap in the Application system, the User must immediately report the findings in writing to the application. Users are prohibited from using this for personal or specific group interests and publishing to the general public.
Users can submit questions, complaints, and complaints connected with Personal Data or Personal Information on the Application. Any feedback, suggestions, and or findings provided by the User regarding the application are not considered confidential information. The Application reserves the right to use this information freely without limits. Users are prohibited from abusing the findings so that they can affect the operation of the application.
The application will verify Personal Data by referring to the Personal Data stored in the Application system. The application has the right to reject questions, complaints, and complaints submitted if the Personal Data has not been verified.
Any additions, reductions, and changes to the Application complaint channel will be informed later through Application notifications or other official channels.
